Before you submit a request, all patients must meet the following requirements:
Have met with your Superior provider in the last 3-months
Have submitted labs to your Superior provider in the last 3-months
Have maintained good patient standing with Superior Health & Wellness
If you don’t meet these requirements, please schedule an appointment with your superior provider and complete another set of labs.
