Need a Refill?

Before you submit a request, all patients must meet the following requirements:

  1. Have met with your Superior provider in the last 3-months

  2. Have submitted labs to your Superior provider in the last 3-months

  3. Have maintained good patient standing with Superior Health & Wellness

If you don’t meet these requirements, please schedule an appointment with your superior provider and complete another set of labs.
